In 2018, an autonomous Uber vehicle struck and killed a pedestrian in Tempe, Arizona. This tragic incident raised critical questions about the accountability of self-driving technology. Investigations revealed that the vehicle's AI had failed to recognize the pedestrian in time to prevent the accident. The incident raises discussions around Uber's responsibilities, the manufacturer of the vehicle, and developers who designed the AI algorithms. But ultimately, while the driver was not in control of the vehicle, the broader implications of accountability fell on Uber and its safety protocols.

But some Amazon investors have criticized the technology, Rekognition, over its alleged biases in particular against women and people of color. Microsoft rapidly pulled Tay offline, acknowledging the failure in the design and oversight of the AI.
